Annual Reviews

If you suffer from any of the following chronic conditions, we will invite you to make an appointment, with the nurse, for a review, to help manage your condition:

Asthma

High Blood

Pressure Diabetes

Heart Disease

For Diabetes, High Blood Pressure and Heart Disease there are 2 reviews: We will send you a reminder letter for your annual review around your birthday and then 6 months later invite you for an interim review.

For Asthma, there is just one annual review and you will receive a letter around your birthday.

Please contact Reception to make the appointment. It would be helpful if you provide details of your letter so we can make the correct arrangements.

If you feel you are not due to have a review when you receive your reminder please let us know.
