Birthday Annual Reviews

If you suffer from any of the following chronic conditions, we invite you to make an appointment, with the nurse, for a review, to help manage your condition:

Asthma

Diabetes

Heart Disease

High Blood Pressure

High Blood Pressure Monitoring requires 2 reviews. 1 annual review around the time of your birthday and an interim review 6 months later.

For Asthma, Diabetes and Heart Disease there is just one annual review to be done around the time of your birthday.
