how to register

To register with the practice you need to live within our catchment area, complete a Registration Form and a New Patient Questionnaire for every member of your family. These can be collected from the surgeries or downloaded from this website on the Practice Forms page.

Once the forms have been completed please return them to the surgery or email them to admin.northendsurgery@nhs.net.

If you are on repeat medication you will need to see a Doctor before your repeat medication can be re-authorised.

We request the forms to be handed in 48hrs before you need to see a Doctor, but we understand that due to the nature of being ill sometimes you may need to see a doctor sooner. We are happy to arrange this, however we will need the completed form to be able to register you on the computer.
